I need a sign out field on a guestbook form

Hi, 

I'm trying to replace a paper guest book in my college's reception. I have all my fields set up and a time stamp field for people signing in. 

I've searched forums for help building guest books which dozens of people have asked about and your 'support' just seem to reply saying "look at this thread" and the link then doesn't work. 

Can I do this or not?
