Why is my account suspended?

    Hi all

    I do not know why my JotForm account is suspended.

    When I try to log in as usual, and I get this message: Login failed - SUSPENDED

    After

    Please, can you help me?

    Upon review of your account I found that it was suspended because of form: https://www.jotform.com/71422285816356 is requesting banking information on plain field texts and upload of bank statement.

    If you are using this form in order to collect payments I highly recommend you to use one of our "Payment Processors" in order to avoid any violation.

    Or if you are using the form in order to create a direct deposit or a refund you need to add our two mandatory widgets that allows your customers to authorize the collection of their Banking Information:

    1. Add the Terms & Conditions Widget  for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    I have reactivate your account for you to make the appropriate changes as soon as possible.
