Just created my first form and I've used the Source Code in order to use the prepopulating the fields.
I've tested on all cross platform browsers (Mac and PC --- Safari, Chrome, Firefox and IE) and found out that IE is not validating the forms properly. When you click submit on the form it just post the form and skipped the form validation.
I've tested it through Preview on jotform.com and it works fine until I use the source code.

The issue may have to do with a conflict between the scripts of your page.
We do not allow the collection of sensitive information (credit card numbers) through our forms.
Please use one of our secure payment gateways for this purpose. Paypal Pro, Authorize.net and Stripe allows you to do this in a safe manner.
This will add credibility to your site and prevent your account from being suspended.
