Can i Create custom forms

I need to create a registration form for after school programs that need to include some customer questions. and at the end they will need to be able to make payment.  (Every school will have their very own form. 7 schools total.  Prent and child info and payment.) Will these forms link into my square database?   Name / Payment info and all?
