    We do not support script embedding into the forms due to security reasons, but since there is also a no script image tracking pixel provided by Facebook, you can try to add it to your form. Although, this solution have not been tested yet.

    1) Copy your tracking pixel image code between tags.

    Example:

    <noscript>

    <img height="1" width="1" style="display:none;" src="https://www.facebook.com/tr?id=1234567890&ev=PageView&noscript=1"/>

    </noscript>

    2) Add it to your form with a Text (HTML) tool.

    I did check your last edited form and it seems you are using cards form layout. Please note the welcome page of cards form does not allow to add any html code other then the text content.

    I would suggest you to please add "Text" field in a new card / page and add the pixel code.

    Hope this helps.

    Do get back to us if you have any questions.

    So it has to be on a page all alone and not on a question page?

    In a Cards layout, which allows only one question per page so definitely in an individual page you should include my colleague's suggestion.

    Give it a try and let us know the results.

    Thanks.
