Hello, I am trying to build (or preferably just clone) the form that Jotform has created. Please see attached image of the form i am referring to.

You can use the form templates from the Form Templates gallery

Hello, thank you for your response. However, I believe you may have misunderstood my inquiry. I already understand how to use the templates. But what i'm looking for is a template that has two separate forms side by side (as in the screen shot I had attached to the original post). Is it possible to use this exact template (as in the screen shot)?

Thank you for the explanation. The screen you mentioned on the screenshot is not a form template.
It is the user interface (UI) in the Form Builder. So you cannot actually use it as form template.
Simply put, it is not a form but a GUI.

Is it possible to use this exact template (as in the screen shot)?
This is doable also on a form. What you can do is use the Iframe Embed widget on a form.

Then add 2 forms within the 2 iframe widget on a form. You will need 3 forms at the least.
The main form width will need to be extra wide to be able to accommodate the 2 iframes with embedded forms side by side.
