Can't submit form, being told "Incomplete Values"

Hello.  I'm trying to publish form https://form.jotform.com/82475073893164 and cannot get it to submit when I do a test.  I'm filling out all of the required forms.  I have gone back to the builder and double checked everything and still can't figure it out.  I'm considering a premium account, but very frustrated I can't even get a free one to work right. I've tried from multiple computers and browsers.  

Maybe it's something simple I'm missing.  Please help. 

I made a test submission on your form and it went through just fine. I did not get any error.

I also cloned your form, made all the fields required then performed a test submission after that, it's all good.
