I've removed this integration 3 times because it mysteriously broke (see odd URL) but every time I go back to re-integrate, the old integration remains in place.

Hello Again and I do apologize for this problem as well. I believe this may be related to your other problem as mentioned in this thread here http://www.jotform.com/answers/158622-Integrations-lightbox-style-popup-inaccessible-at-top-of-window#1.
Something I would like for you to try though to see if it helps to correct this issue because it is possible that the cache and cookies that were saved to your browser which may have become corrupted causing further problems. These relay information between us and your account and your computer. To try to correct this problem what I would like for you to do a Hard Refresh. To do this please log out, clear your browser cache, then press F5 which will close your browser entirely and restart it again then you open up our website followed by logging into your account again. After you do this try to remove the integration as you previously were trying to do. This may also help to correct the issue with your other Integration Wizard Popup Problem.
Please let us know if you are still having problems.

Could you provide instructions for MacOS? Don't think F5 will do it on this machine.

OK I just went to a completely different computer that has never visited Jotforms before and used an entirely different set of tools (Windows 7, IE9). Same problem. I go to integrations, REMOVE INTEGRATION (which doesn't work, broken URL, as per the first post). Exit Jotform, clear all caches and browsing history with cookies, launch app, go to Jotform, Log In, go to form, go to Integrations Wizard, and it STILL says "Connected to Google Docs."

May I suggest that you clone the form and try the integration on the cloned form.

That means collected data on the original form will be separate from subsequently collected data, then?

Yes, but you can copy and paste the current collected data on the previous spreadsheet to the new one so you don't miss a single submission

UPDATE: I've found that if I click the red "Remove Integration" button, then immediatly click the INTEGRATIONS menu and go straight to Google Spreadsheets to set up a new integration for the same form, it suceeds.
Oddly, what causes the integration to persistently remain attached to the form is clicking SAVE or doing anything between removing the integration and re-doing the integration. Strange, unexpected behavior.

I had also encountered this odd behaviour in the integration process before. What I had learned about it is, that it had to do with the current performance on my internet/network. Latency had to do something about it (at least on my end).
But I also learned the procedure that I am now using since then to prevent the inconsistency and unsuccesful integration.
What I always do is, I make sure that I am currently logged-in already to my Google accounts (gmail, and and google drive also opened) on the same browser. Then, I open/edit my jotform form and do the google integrate process. This had been consistently working since then.
