Upload the PDF on the cloud like Google drive and share the file link here. I have made the thread private so it will be secure. Using this we can test the PDF upload on our end and assist you to get it working correctly.
Do share the form URL on which you are embedding this too.

Thanks for sharing the PDF document. I have successfully embedded it on a test Form here: https://www.jotform.com/82634334671963
In addition, I have cloned your form and embedded the PDF here: https://form.jotform.com/82635190471962
I used the latest version of Chrome. It could be an extension of Chrome you added that is preventing the upload or a configuration you have set up for Chrome.
I notice you tested a different browser and it worked. If possible, try it out on a different computer using Chrome and let us know how it goes.

As per your request, I have embedded the PDF on your form: https://form.jotform.com/82626961660160.
The Widget can be repositioned by selecting and dragging it to the desired position. In addition, clicking the properties icon can allow you to edit the view options for the Widget:
We apologize for the inconvenience you are having uploading the PDF using the Widget.
As recommended, try uploading the PDF using a different device and let us know the results. You can post screenshots to let us know where it fails. This is so we can reproduce the issue on our end and help fix it.

Hello,
It seems that there are multiple issues on your side and in Chrome browser only, which are blocking our servers. Have you installed additional plugins/extensions for Chrome with the latest update?
I can also suggest to try clearing cache and cookies of the browser to see if this can make things better.
