The custom background does not show on our form

    Good Morning!

    We have been using your site for well over a year and have several company forms that our field staff use and it has worked beautifully until about a week ago. 

    For some reason, our forms are now malfunctioning. The custom background does not show when the form is pulled up for use which isn't ideal but not a crisis, however, much of the programming we had in the form no longer works either. 

    I'm wondering if this issue has something to do with the recent upgrades you did to your site? How can we get this resolved as soon as possible? 

    Someone on my team attempted to contact you last week; however, we have heard nothing back and are quite concerned.
