How can I make a to and from date?

I need to add two dates per line for an immigration question. So the lines need to look like this:

Entry Date: dd/mm/yy        Exit Date: dd/mm/yy

Entry Date: dd/mm/yy        Exit Date: dd/mm/yy

(option to add more lines - if possible)

You can either use a Date Picker field with date rage option which allows you to capture Entry and Exit dates on the same line.

However, looking at your requirements I would suggest you use Configurable List with Calendar Option. With this, you have the flexibility to add the list on and on.
