Hi,
I may have changed my online application format too many times. When the answers are delivered to my email address, the right answers are there but the wrong question is listed.

Check the body of the email notification to ensure the correct field labels are entered. Here is more information on how to update the contents of an email notification or autoresponder:

Alternately, creating a new notification email would create a new email with all current forms fields by default.

If you go that route, make sure to delete the current notification or multiple notifications would send with each submission.
