How do I tag form fields to be read by a screen reader?

I am trying to figure out why a JAWS screen reader is only reading some of the form fields? It is not reading the field title for the radio or checkbox fields.

Please help.

I am not sure how the JAWS screenreader works. I am downloading the Free trial to test it. Please allow me sometime to test it with your form and get back to you with relevant information.
