All of a sudden my configurable list is not showing up in my form. Please assist.

I have checked your JotForm and see that the configurable list is being displayed normally at my end.
Are you still experiencing any issue with the form? If so, please try opening the form in a private/incognito window or a separate browser to see if that works.
