It may be possible that your server is rejecting the emails from us due to DomainKeys issues (the notification email says it came from the email entered by the user, when in fact it was sent from JotForm's server).
Try configuring your form to use email@example.com as its Sender Email.
You may use the form's email field as the Reply-To Email so you'll be able to reply to the form submitter directly, if needed.
On a related note, your form contains two similar notification email. We recommend keeping just one of them as multiple copies of the same email may trigger anti-spam filters on an email server.
