I have an intake form I can no longer find in my templates

Is there a way to get back a form I made even if it no longer shows in my templets made? The link on my website wasn't working and so I went to check in jot forms and it's gone. I am not sure why. 

I have checked your account and see that there are the following 2 forms in it.

Client Intake

The forms seems to be loading normally at my end. Were you able to restore the forms or fix the issue? If you still need any further assistance, please let us know. We will be happy to help. 

Correct but I have made another intake form for Childbirth Education client
that use to work and now I can’t find. Not sure how that’s possible or if
it’s possible to retrieve it.

Unfortunately, we cannot find a form based just on a form name, do you have any URL (even email received from it(submission))?

Or any other related information that might help?

Unfortunately, that form URL doesn't exist, so I'm not sure how we can find your form otherwise, did you receive any submissions from that form?
