I have an email field created in both Mailchimp and Jotform and now it is asking to "Please add email field". (screenshot attached)
After clicking 'Add New Option', the left dropdown offers 'Full Name' & 'Phone number'.
The right drop down offers nothing, a long blank list.
I have removed and reconnected integration
I have created a new API key from Mailchimp
The form in Mailchimp has only 4 fields...
The form in Jotform has only 3 fields...
Can you please advise what to try next? Thank you..

Can you please double check your API key if correct or try to create a new one.
I tried this on my end using your form and my mailchimp api key -- I was able to integrate it just fine. (See the screens below.)
Update us if the issue persist after doing all the necessary test.

Having same exact issue and have tried deleting and re-adding key, deleting field and re-adding. Still nothing.
