I just notices that I'm not receiving any of my forms to my email? Can you look at my form above and see what's wrong.

You delete one the email notfication that is not sent by email@example.com but other than that I see no issues with how you have set up your form.
Our email log tells me that email notifications from this form are being accepted for delivery by your email service provider.

Are you able to ask your email provider where your email notifications are going? You can even give them a message id: 0000013bbc4c8202-e16f9e6a-35d0-41f4-8c34-4b1b588166ec-000000
