Your form isn't working because you don't have the correct DOCTYPE declaration

Please replace it with the following one:

    <!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">

Alternatively, you can use the iframe code, to prevent the issues

Additionally, I just found out that your account has been suspended automatically by our Phishing detection system , due to your form Contact Us , which seems to be a login forms and this might violate our terms of use . However I see it's more likely a setup username form.

I have unsuspended your account, but I cannot assure you this will not happen again

Please understand that Phishers will use our form builder to make forms to catch passwords or credit card sensitive information into plain text fields , therefore violating our Terms of use.  So, in order to control phishers we have implemented this system as we have millions of forms already hosted into our servers. Fighting with real phishers is an everyday task and cannot be manually done in most of the cases
