Form is not displaying correctly on iPhone and Andoid devices

I created a form but I can not use it on the Iphone and Android is coming out with the words exchanged.

How to adjust this bug?

Please be noted that the forms created using JotForm are mobile responsive by default. I believe that you are referring to the form https://www.jotformz.com/form/80503873641658 in your account. I see that the form is now using Card form layout and should be displaying responsive on mobile devices.
