PayPal Pro: Unable to submit a form

I've completed all of these steps and it is still not submitting or redirecting to PayPal. Please help!

I inspected the forms in your account, but I did not find any form that is integrated with PayPal Pro. 

To which form is this issue related to? Was there some error message shown when you try to submit the form? If so, please provide us a screenshot of the error message.
