Copied form doesn't have the layout of the form being cloned

Hallo,

A time ago, I did made a form in a quick nice format (with background, stars en next questions button...)

When I make a copy of it, only the questions are copied.  How can I do to also have exactly the same format?

Can you tell us how exactly did you clone the form, please? If you have cloned the form using the "Create Form" button, there is an option that will let you choose the layout of the clone version of the form. But, if you have used the "Clone Form" in the "More" drop-down menu, then the clone version will have the form layout of the form being cloned.
