How to open JotForm from a section in the header of a webpage?

    I presently have a page at http://www.ketopet.com/TEMP/draft2.html. I want to change the page so the "Sign Up for our Newsletter" link opens to a lightboxed Jotform (Sign Up for Keto Newsletter) instead of a new web page as it does presently (http://fitwarranty.com/ketopet/newsletter). 

    The current coding is as follows:

    I want to somehow change that to somehow include code so that the lightbox replaces the webpage with some type of click on statement, I guess. JotForm's normal code is as follows:

    Assuming that this is your lightbox codes, place this codes where the newsletter signup button is placed;

    Notice the bold red text, you can replace it with HTML Image tag to point on your header newseletter button.
