In fields like phone number and address, how do I separate out the information when transcibed to the new form? Right now, the whole phone number is getting copied into the area code box and the entire address is getting copied over to the street address field.

For the address field, here is a sample breakdown of the URL parameter.
For the phone number:
?mobilenumber[area]={mobilenumber:area}&mobilenumber[phone]={mobilenumber:phone}

You need to use separate URL parameters for each input.
For the phone number, follow this pattern:
https://www.jotform.com/80896386001157?yourPhone[area]=12&yourPhone[phone]=3456789
As you can see, the area is populated with "12" and the rest (number) with "3456789".
For the Address field, follow this patern:
Phone number and Address:
I also suggest you disable the Continue Forms Later option in the form settings as this can create conflicts between the URL parameters and browser cookies.

fields with the values you've inputed in the url, but I want the form to
prepopulate based on the most recent response.
I was able to get it to work with the name field

but when I did the same thing for the phone number and the address, the
information would not parse out into their respective text fields. The
answer would get copied over and clumped together in the first text field.
What should the second part of the url be?
yourPhone[area]={yourPhone:area}&yourPhone[phone]={yourPhone:phone}

As my colleague has already mentioned, the issue could be caused by browser autofill set to on for your form. I assume that you've once got the incorrect input when testing the form and this value was saved to your browser. Please disable this and test your form again: How-to-Enable-Auto-Fill-on-Forms

I am almost there. All the fields I want prepopulated work except for the
address fields.
This is what I have:
address[addr_line1]=address:addr_line1&address[addr_line2]=address:addr_line2&address[city]=address:city&address[state]=address:state&address[postal]=address:postal
Am I doing something wrong?
