Please let us know what device and browser you are viewing the form on. I have tested the form on desktop and mobile and it is showing as required. Is your form embedded on a website? If so provide the URL too so we can take a look.

    Hi,

the form is embeded on a site using the floating button pop up embed option, here's the test site link

    http://chandalynnrecoverycoach.com/

Its perfect everywhere else, just on the button pop up in the corner of the screen

Thank you for additional information. Card forms are not very flexible as to customizing, so I will attach a ticket to this thread so our developers could check this issue closer and provide a possible fix.

Meanwhile, I can suggest to try another embedding option, for example LightBoxHow-to-Create-a-Lightbox-Form

Also, you can temporary disable HIPAA logo: How-to-remove-HIPAA-Compliance-Badge-on-the-forms

Our apologies for any inconvenience caused.
