Form has suddenly changed and added a large header to the top

There has been no changes.  Suddenly this morning now there is a large empty header space at the top of the form that is causing the job ticket to go to 2 pages and also it duplicates the name.  I do not see that section in the in the editor.  The form looks right but the job ticket is off.  See screen shot, highlighted area has suddenly appeared.
