I've had my form embedded in my website (zenfolio) for years now with no issue, but suddenly it's not working. I've tried re-embedding the link (both via the normal embed code and using iframe however it is still not working? Any suggestions? When I embed it is just showing a small square....

I have checked the web page provided and unable to find any form embedded. Did you remove the web page? Could you check once again and let us know if you are still experiencing the issue?

That is the problem, I embedded it but its not showing up? This is a new issue, I've never had a problem for years?? Here is what it looks like after I embed it on my side...

By unable to find the form, he was referring to the absence of JotForm code in the page. I checked the page and searched for any JotForm embed code and was also not able to see any.
If you can edit the source code of your page and add in the form iFrame code, it should work:

As I check the form embed code on the web page, I see that the width is somehow set to 1px.
Please try changing it to 650px in the embedded code so that the form should be displaying properly on the web page. Here is my test page with your form embed code.
