Calculate Form Results

Hi, 

I am trying to create a self-assessment online as a form of lead generation. This is the functionality I am looking for:

-Collect name and email

- Ask assessment questions

-Provide score results after assessment to be displayed on site and sent via email to user. 

Do you have a tutorial(s) on how I can accomplish this?
