Upon review of your account I found that it was automatically suspended (autosuspended) by our Phishing Detector Tool because of form: http://www.jotform.us/form/82906273673161 is requesting password.

    https://www.screencast.com/t/xUWHZeaY4Sqf

This is not allowed, since the collection of login credentials needs encryption along database administration and our forms do not offer this method.

I have deleted the infringing field and reactivated your account.

You may continue developing your forms according to our: Terms of Use

If you need further assistance, feel free to contact us.

    Thank you

OK I tried to copy an existing form and that must have been embedded. I
will start with a fresh form so I dont have those issues. Whe I try to
login now the box just sits there. Not sure if I am really logged in or
not and dont want to create a form that might not be saved to my account if
I am not logged in. Any help would be appreciated. Thx

The form my colleague referred to is hosted in the account username "westshorescuba", this account is registered with the email address "info@westshorescuba.com".

If you forgot your password, simply click on the "Forgot password?", and enter your username. The reset instruction will be sent to the registered email address.

I can also see that you have created a form under a guest account, which is: http://www.jotform.co/form/82906628473870

If you want to, we can move that form to your registered account username "westshorescuba".
