    The form is - https://form.jotformeu.com/82795047853368 which displays correctly.
    There is a certificate error after pressing submit:

    We then get the brightcloud.com blocking access:

    I am not the author of the website and do not have access to the support forums to add screenshots to this case.
    I am someone trying to submit a form as described but submit.jotformeu.com is blacklisted by brightcloud.com
    You need to improve the reputation of submit.jotformeu.com with brightcloud.com - https://www.brightcloud.com/tools/change-request.php

    We may need to clarify with you the link to the brightcloud.com where the form is embedded. Also, can you tell us what type of device, browser, and/or connection that you are using when you attempted to submit the form?

    Hi, 

    We requested reputation change from BrightCloud but we got this answer: 

    "We have reviewed submit.jotformeu.com and determined that it does not need to be changed at this time based on BrightCloud’s classification criteria.

    It is currently classified as Computer and Internet Info in the BrightCloud Service and available in Database version 5.717."

    We are trying to reach their support for reconsideration. Sorry for the inconvenience.
