I am not finding basic form elements for my form

Hi I just want simple options to create such as multiple choice with adding text, etc. (please see screen shot I've attached and compare it with the form as it is now on my website- also attached).

Somehow I am not finding these simple options but assume they exist and I just don't know.

Checkbox is called Multiple Choice in the Form Elements.

Example: You can also add the Other Option to insert a textbox on the checkbox field.

For the Textbox, use Short Text Entry.

There are also Widgets that you can use like Multiple Text Fields
