I have checked the e-mail address on your form and found that it was in our bounce list.
Please follow this guide in order to avoid this in the future:
Please whitelist our Jotmails ip addresses or ask your email service provider to do it for you.
Also, a good suggestion would be to set the sender address to email@example.com on your form notifications.
