    Hi, my account has been autosuspended. Please help.

    Upon review of your account I found that it was automatically suspended (autosuspended) by our Phishing Detector Tool because form: http://www.jotformeu.com/form/82924746384367 is requesting login credentials (username/email/password)

    Gathering this type of data infringes our Terms of Use, as this is only allowed through encryption along database administration; and our forms do not provide this kind of service.
