I need a form to get the username and passwords for certain companies.

Hello, 

I need a form made for when we do A/V work that i can send the customer to get their username and passwords for certain companies. Is there a way to get 3 text field boxes in one box. (Company,Username,Password)? Also with the ability to add more lines if need be?
