Experiencing form issues with the web form, and PDF version.

why when I send the form i created it sends only a white bar?

also, when I download the pdf, it has an extra white page attached to it. 

It also changes the fonts and spacing. So frustrating! I can't just make a normal pdf form.

What do you exactly mean? When does the white bar appear? I assume that you are referring to the Thank You page not being displayed when submitting the form, instead a white bar appears, am I correct?
