    How can I show the headers when generating PDF of a completed form?

    That's not possible using Get PDF button on the submissions page but there's a trick using Google Chrome Browser's Save PDF Print Option when you print the submissions. Check this visual guide.

    You need to show the headers and texts first, here's how:

    After that, Click on Print button near Get PDF button. Be sure to use Google Chrome Browser. After clicking print button, the print preview will appear, just change the destination to Save as PDF to save the submission.
