    Hi,

    Thanks for that details.

Is there any form URL you've received on the notification message? Something like this: http://www.jotform.com/10732640770

Most probably that user created a form at JotForm and used it for abuse or phishing. If you can provide the form URL or the website URL from where it is embedded then we can help you resolved this issue. We can easily suspend the form here and ban the user's IP Address.

JotForm has a zero tolerance policy when it comes to illegal online activities.

    We'll await your reply. Thanks!

    Hi,

Sorry for my misunderstanding. I just realized that you are talking about submissions on that form which uses your name? Unfortunately, there's nothing we can do about that. Our form is just a medium tool where users can submit and the form owner receiveds its data. You should probably asked the form owner as to how it happens. That's most probably done by guest viewers on that website. We are completely blind to this issue and we do not hold any records on millions of form submitters over the internet so we can not provide any further details.

    Thanks.
