Form to create standard sell sheet for an item

What would be the best approach to create a standard sell sheet for vehicles for my company utilizing a form that publishes to a PDF?

I'd like for the user to upload a picture and then select options and fill in text fields. Is it possible to have the Picture automatically resized to a "standard" size and the selected options then formatted onto the PDF?

The uploaded images will be resized inside of the PDF report by default. It is also possible to customize PDF reports with the PDF Editor.
