What is the easiest way to ask users to add more entry?

We are trying to use conditional logic to get members to add additional members to their roster when they apply for membership.

We have set up fields for "Do you need to add additional members?  And then if yes, three fields should appear with Member name, title and email.  We want to add groups like this for up to 20 members.  What is the easiest/most efficient way to keep adding "Do you want to add additional members?"  And then, if yes, show the three fields, if no, skip to the last page?

You can use Configurable List Widget to add more members. First field would be the regular single choice field and set a condition that will show the Configurable List Widget if the answer is "Yes".
