Reset password

Hello,

I created an account and a form about 6 months ago, but now I can't remember my login/password.  I have tried to send a password reset link but nothing is getting sent to my email address.  I've tried several different times to get it reset over the last two weeks.  I'm hoping someone can help me get back into my account so I can edit my form.

Could you please provide the Email you used to create the account. There is no account existing for the email you provided in the query: agar**ner@veritassc***l.net. Alternatively, if you remember the username, you could provide that as well. Using this information we can be able to look into this further.
