Good Day
Please explain:
I would like to ad on my jot form the functionality to upload a copy ID document for my clients.
Please explain if it is compulsory to include the Signature widget as well as the Terms and conditions widget.

Hello,
As our colleague has already mentioned on email, digital uploads of any kind that contains sensitive information are not allowed.
Be advised that this type of collection is not allowed since our upload buttons are not offering encryption for this type of data.
Might you need the following information, we allow the collection of the numbers with the appropriate signature widget and Agreements :
1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities
2. Add a Signature widget where your users sign and agree to provide such sensitive information.
We will gladly assist if you need any further help.

As per the above mentioned inquiry.
Please advise me with my form I would like to publish. I have inserted the
Terms and conditions section, Uploading of Docs widget, The Signature.
But need help with the wording for the Terms and condition page.
Will you be able to help me with this?

Hello Charlton,
Unfortunately, we do not have text samples for Terms widgets. Basically, you should ask respondents to agree that they are willing to provide such sensitive information by checking widget's checkbox.
Regarding your form http://www.myjotform.com/form/83081326764560:
It seems that you have still added an upload field for ID documents. I would like to mention again, that this type of fields is not allowed. You should replace it with Short Text Entry field for users to type in their ID number, not to upload it:
Please remove the field to avoid suspension by our phishing detector.
