I have been able to submit your form with no issues. Am unable to replicate this error on my end. Could you provide what browsers this is occurring so we can do further tests?

Awesome! She was using Internet Explorer but was able to submit finally. This happened to another individual but I did not receive a response as to the browser that was used

I have gone ahead and cleared your form cache. Do let us know if this occurs again and inform the user filling the form to try submitting the form once more.
