We recently upgraded our corporate gmail services to G Suite. After the upgrade our jotform response emails are now showing in our inbox as a suspicious phishing email.
I have made several attempts to work with Jotform support to resolve the issue.
The first suggestion was to go into the form Settings, Emails, Jotform notification email, Recipients, and change the Reply-to Email to "Please Select" and then Save.
We can perform a test and the result is a form that does not have the phishing warning.
However it was discovered that 2 minutes later, that setting is reverted BACK to Email. All attempts to saving and it staying does not happen.
We'd like for this to be resolved. Thank you.

Does the issue continues after click on "Looks safe"? It appears that the warning is related to your custom sender:
Try using "noreply@jotform.com" instead of "support@cardhoarder.com". Let us know if you need more help.

Also, make sure to check the spam folder, and the form notifications as NOT SPAM if you find them there.
