Is there a way to upload a pdf and add form fields?

I already have a pdf document.  Can I upload the pdf, add fillable fields - creating a fillable form, so that we can embed a fillable pdf into an email and send a link to potential employees before their interview. 

A PDF file can be embedded into a form using a PDF Embedder widget:

Then, you can add the fillable fields to the form. However, please note that the form fields will not be integrated with the PDF file.

It is also possible to automatically generate a PDF document with filled information upon a form submission. The PDF document template needs to be created through our editor (you cannot upload your own PDF template), see:
