Can you please share with us the Form URL so we can check your settings. Maybe the field variables have not been included in your form notifications or the field variable have change and that needs you to update it.
You might want to consider deleting your current form notification/auto responder by following this guide: How to Delete Notifications/Autoresponder and then re-create the by using this guide: Receiving Emails on Form Submissions and Creating a Form Autoresponder
