I am trying to come up with a spreadsheet with 3 columns that calculates the selection

I am trying to come up with a spreadsheet with 3 columns.

Column #1 is clickable or radio button

Column #2 is text

Column #3 is blank but a number (a price) is inserted if the radio button is enabled or clicked from column #1

It's going to be a menu and invoice so the fields must be able to have calculations.

Is there a widget or template that will work.  I've looked at all the spreadsheet widgets and can seem to be able to accomplish what I need

I would suggest using our basic fields such as the radio button, the text/text area box, and calculation. You can align these fields to create a column like layout.
