Autosuspended

I just tried to create a form using the event signup template and I can't login to save my work because I get an autosuspended message. Please help

Your account was suspended because your form asks username and password. Note that we do not allow this type of information to be asked on the form.

I unsuspended your account now and deleted the fields. Please do not recreate the fields to avoid this from happening again.
