Can we please talk concerning integrating the form onto our webpage

In regards to embedding your form in your site, you may simply get your form's embed code, that you will embed in your site to display the form: https://www.jotform.com/help/34-Adding-a-Form-to-a-Web-Page 

If the "Embed" code option does not work, try embedding the form using the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code 

Let us know if you need more help, and provide the URL to your site if you see any issues with the form.
