Auto Suspended account?

Hi there, 
I have just tried to set up two accounts and both were suspended while i was creating a form??? 

Please help! 

Upon review of your accounts I found that they were automatically suspended (autosuspended) by our Phishing Detector Tool due to forms: http://www.jotform.co/form/83174726549871 and http://www.jotform.ca/form/83174121839256 are requesting login credentials (email/username/email)

Gathering this type of data infringes our Terms of Use, as this is only allowed through encryption along database administration; and our forms do not provide this kind of service.

I have deleted the infringing fields and reactivated both of your accounts.
