I understand that the PDF report is not displaying all the fields in the report. Could you check if all the fields on the forms are filled correctly in the submission since the hiding empty fields options may have been enabled in the report? Also, could you try creating a new report and see if that works?

Please get back to us if you need any further assistance. We will be happy to help.

    Hello Kiran,

    Thanks for your reply,

When I submitted the form and go to the "View Submission" page then it shows all the label and its entered inputs.1542376525Screenshot from 2018-11-16 19-

Generally, the PDF report displays blank when it is accessed using www while the account is enabled with HIPAA compliance. Could you check if the PDF editor URL is having hipaa in the URL or try using the following URL to see if the submissions are displayed correctly in the PDF preview.

I have cloned your Jotform to our HIPAA test account and my account. I am able to replicate the issue with the form on both accounts. I have tried creating a new PDF document which is also displaying blank. However, as I tried to create a new notification and autoresponder, the PDF reports are displaying correctly. The issue also seems to be with the email notification/autoresponder on the form. Please try removing the existing emails from the form and create a new one, attach the newly created PDF reports to see if that works.

    Hello,

I am having the same issue and all of my clients are complaining. They are supposed to receive a PDF form attachment for a copy of their submissions and they are stating that the PDF are empty. I tried doing a test and I do show that the PDF has nothing. I tried deleting the autoresponder and creating a new one but it still does not work.
